Initial Intake
There is a $315 fee for the initial 90 minute intake appointment. All patients who are new to Petoskey Center are required to participate in an Intake session prior to starting one of our groups.
Intensive Outpatient Program (IOP)

We recognize that treatment can be expensive, a considerable investment of both time and money. As a result, we do our very best to truly serve patients’ individual needs. We work toward helping you achieve your specific goals through our personalized programming. Please call for pricing.
Payment Schedule:
The total IOP fee is due at the time of committing to attend the program unless a separate arrangement has been agreed upon in writing. Due to the importance of participant attendance for the entirety of the treatment program, the IOP fee will not be refunded even if the patient is absent on a given day(s) or chooses to drop out of the program prior to the end of the cycle.
In addition to the clinical importance of full attendance, most insurance companies will not reimburse for absences or partial attendance. The IOP diagnostic code (determined by the insurance industry) uses a “per day” charging unit (1 IOP day = 1 unit). Regardless of your insurance company policy for attendance and reimbursement, we strongly recommend that you attend as much of the program as possible.
Evening Groups
Fees

Unless otherwise stated (e.g., time-limited groups), we encourage patients to consider attending the stand alone evening groups for a minimum of 3-6 months.
Payment Schedule:
Payments for the Evening Groups are collected monthly at the start of each month. We currently accept payment in the form of check, credit card, or bank transfer (Zelle/Quickpay).
Patients must give 24 hours notice of their absence in order to avoid paying the full fee for the session.
Insurance Information
We are an out-of-network provider for all insurance companies. However, patients often have out-of-network benefits that allow them to receive reimbursement for a portion of our program fees. We are happy to complete the clinical authorization for you to be able to use your out-of-network benefits. However, we will not seek authorization unless you request us to do so.
Oftentimes, insurance companies have a strict set of policies governing what constitutes medical necessity for intensive outpatient level of care. Consequently, your insurance company may decide to deny coverage even when you and your treatment team believe that IOP level of care is appropriate. In these situations, your insurance will only reimburse you for what they define as medically necessary.
Given that some carriers require pre-authorization, please make sure to 1) speak to your insurance carrier to clarify requirements and coverage details prior to starting our program and 2) inform us that you would like to use your insurance benefits so that we can make the necessary pre-authorization phone calls.
Please note: We provide superbills to allow you to file your insurance claim for Petoskey program services. We do not file claims directly with your insurance company. It is your responsibility to file all necessary paperwork, including your invoice (superbill) to your insurance company.
